Manage a caseload of young people aged 17–25, undertaking Care Act assessments, reviews and support planning
Apply the 3 Conversations Model to deliver person-centred, strengths-based interventions focused on independence and resilience
Complete mental capacity assessments and contribute to Best Interest decisions in line with the Mental Capacity Act
Develop and commission care and support packages, including personal budgets and direct payments
Lead and participate in multi-agency meetings, including EHCP reviews, Section 117 aftercare planning, and Continuing Healthcare processes
Degree or equivalent in Social Work (BA/BSc/MA, DipSW or CQSW)
Registered with Social Work England
Strong working knowledge of the Care Act 2014
Strong working knowledge of the Mental Capacity Act 2005
Strong working knowledge of the Equality Act 2010
Understanding of Safeguarding Adults procedures and statutory guidance
Familiarity with the Preparing for Adulthood framework
Familiarity with SEND processes
