• Encourage appropriate referrals into the services via professional and self-referral channels
• Utilise an Asset Based Community Development approach that leverages and harnesses the richness of skills, experiences, and opportunities from across the areas and within communities
• Work with key referring pathways to establish innovative engagement techniques that ensure referrals are steady and targeted to key areas and priority groups
• Deploy our marketing and comms efforts to the right locations, via the right medium that ensure efficient, and effective use of resources

2. Develop and establish effective referral pathways into and out of the service:
• Working collaboratively with staff and partner agencies in a coordinated way to reach out and maximise referral pathway opportunities
• Ensure all professional referral routes into the service are simple, clear, and friction-free
• Make sure all referrers understand how to refer, eligibility criteria and scope of service
• Lead on the development and review of outward referral pathways to ensure safe and effective support to service users/participants when requiring additional services
• Establish strong and trusting relationships with a portfolio of wider services / organisations, to provide holistic support to individuals, families, and communities, including (but not limited to), employment services, financial support services, housing options and lifelong learning. Actively seek membership in local networks and meetings.
• Support key referring stakeholders with learning opportunities so they can maximise their opportunity to make their contact count with individuals and build confidence to support and advocate lifestyle behaviour change
3. Manage, lead, and develop key indicators that demonstrate and evidence the efforts, progress, and areas to develop across all aspects of community and partnership working including:
• Cross-partnership case studies
• Engagement with new partners
• Innovative referral strategies, complete with measures of effectiveness
• Capture of learning opportunities deployed across the network
4. Develop and deliver presentations, engaging pop-ups, and stands across the wider network and communities to promote services, including working with the Marketing and Communications Team to produce tailored resources to promote programmes and services, including:
• Attendance at GP Practice protected learning time
• Lifestyle related forums and networks
• Community events and fun days
• Support all to access supervision and ensure all engage in the development of annual Personal Development Plans (PDPs)
• Support the Service Manager in the recruitment, onboarding and induction of team members as required
• Create a culture of further development and constructive feedback with individuals and teams, embedding and ensure behaviours demonstrate the company values
6. Working with other team members to harness, capture, and learn from insight and intelligence sourced proactively and informally via referring networks, partners, communities, and individuals to support continuous improvement and scale service reach and engagement.

Qualifications & Experience
• Line management experience, including management of remote teams

• Extensive experience of developing, and maintaining partnership relationships (internal and external)
• Ability to travel independently within an agreed geography
• Experience of working in a health or social care environment or good understanding of the issues around maintaining a healthy lifestyle
• Experience of creating and deploying a strategy
• Experience of report writing and communication of data and outcomes
• Experience working with community partners and stakeholders
