Job Description

About the Role

As Care Coordinator, you will be the vital link between our service users, their families, our care assistants, and external professionals. You will ensure seamless scheduling, quality compliance, and outstanding communication, all while helping us grow as a new provider.

This is a hands-on role in a small, ambitious team. You will work directly with our Registered Manager.

What You'll Do

Liaising with referral partners, promoting new referrals, and processing incoming referrals.

Scheduling: Create and manage weekly rotas; handle last-minute changes; ensure all visits covered

Care plans: Coordinate assessments, reviews, and updates; maintain digital records

Quality: Monitor visit completion, incident reporting, medication alerts; spot-check care delivery

Compliance: Prepare for Care Inspectorate scrutiny; maintain staff files, training records, SSSC registrations

Communication: Liaise with families, GPs, district nurses, social workers; handle enquiries and complaints

Recruitment: Support care assistant recruitment, induction, and retention

Business growth: Contribute to local authority applications, service development, and reputation building

What We're Looking For

2+ years in care coordination,

scheduling, or team leading in domiciliary care

NVQ/SVQ Level 3 or 4 in Health and Social Care

Full UK driving licence and own car

SSSC registration (or working towards)

Exceptional organisational and communication skills

Experience with Log my Care, Birdie, or similar

Proficiency in Microsoft Office and digital care systems

Knowledge of Scottish local authority commissioning

Understanding of Care Inspectorate standards and SSSC requirements

Existing professional network in East/West Dunbartonshire

Right to work in the UK
